Microthane breast augmentation – Fact Check 2. Is it good for sagging breasts?

Hello, everyone. Today I would like to share about microthane, i.e. microurethane shelled implant and how it is effective on sagging breasts.

Sagging breast is caused by three dimensional shape of the breast, therefore, tissues such as Cooper’s ligament lose strength. In most of the time, when a woman gets pregnant or does breast feeding, the breasts swell causing elongation. After the pregnancy or the breast feeding is complete, the breasts lose volume and become smaller. The ligament tissue cells are not rugger therefore, they cannot go back to the original shape. This causes the original breast tissue cells to fall downwards and this is the starting point of the sagging breasts.


The original breast tissue falls downwards, but the skin tissue cells still have the original level of elasticity. We call this glandular ptosis. Such breasts would not have much upper volume but the areola is placed somewhat above the inframammary fold.

As time flies and you get older, the skin loses elasticity and the strength to hold the composition leading to a change in the shape of the breast. This is a little different from glandular ptosis in that the areola drop down below the inframammary fold and you cannot see the white part of the skin from the front side.

There are many patients who consult on what to do during the glandular ptosis stage. A lot of these patients have spent few years after the pregnancy and they have loss in upper volume of the breast with a feeling that the breasts are sagging down with a loss of volume compared to before marriage. At this stage, we need to complete a few tasks in order to recover the breast volume, regain the elasticity and fill up the upper volume.


First, we need to make sure that the implant does not collapse but stand still filling up the upper volume of the breast. Second, the breast tissue must stick to and hold the implant. If the skin loses strength, the implant would fall downwards causing the ptosis to be worse.

In order to achieve these goals and not fall back to the sagging stage, we recommend form stable implants that can be stably place on the muscle. The interface point can become loose therefore we need to provide the tightness by dual plane method.
But, there are conditions in using the form stable implants and applying the dual plane method. First, the skin elasticity must be maintained and the original breast volume should not be too big. Also, we cannot use too heavy (big) implant.


If your skin elasticity is not reliable and you want to use large sized implant, then I would recommend polyurethane to you.

When we use the polyurethane implant, it immediately sticks with the bottom (bone) perfectly. No exterior pressure or strength can change the location. It is very applicable for breast ptosis.

Microthane implant firmly holds the position as soon as it is used. This position does not change at all. The skin layer is supported by this implant therefore we can avoid breast ptosis. (Dual plane method must be applied)

As a result, this is very effective in filling up the upper volume as well. Microthane never sags downwards.

There are some who question if the textured implant that sticks to the skin leads to sagging. The textured implants do not have all the power to stick to the skin. The textured implant would slip away if you widen the pocket with your hands. Such stickiness is not enough to hold if there is much gravity.
 

What happens in the second type of breast ptosis? A case where the breast tissue and the skin all sag, i.e. natural ptosis. We need to provide solution on the skin extending. The usage of the implant cannot solve the problem. We need to cut off the skin surrounding the implant. We need to apply breast ptosis correction. Implant itself only cannot be a solution.

Let’s summarize. There are many situations where we can solve breast ptosis with the usage of microthane implants. This can be the best method for glandular ptosis where the skin does not loose elasticity 100% and you want to fill up the upper volume. This is the simplest of all methods and best method for improvement.

Sagging breasts or breast ptosis is divided into two types of Nipple/areolar ptosis and Glandular ptosis..

Most patients suffer from these two types, and those with severe cases need to apply vertical incision lifting to experience positive result..



But what if the breast ptosis is not severe. Let’s say you are only suffering glandular ptosis or very small degree of it, and you are very young and still have some skin elasticity. In such case, you do not need vertical incision. You just need the lifting correction.

If you only have nipple/areola ptosis and do not have the glandular ptosis, then you would just need to fix the derma and change the location of the areola to have good result. Glandular ptosis need the lifting correction against the force  of gravity, therefore you need to use a technique that pushes up the breast to the wanted position.

Plication mastopexy that I want to share about today, is an effective method that leaves incision only around the areola causing no vertical scar and fixing the breast ptosis.




Incision must be done on the areola and we need to desquamate the outer skin and the glandular by separating the two.

Lifting will only be successful if the glandular itself is pushed upwards to the position that you want. Plication, as that of vertical incision method, is applied in push the glandular at the bottom part of the breast. We need to conduct overlapping suture on the upper part of the breast as if you are walking up the stairs.



We are actually repositioning the breast glandular in such as way that could make the breasts look better. Then we apply purse string suture and complete the procedure. This whole process is time saving.

The biggest advantage of this procedure is that it uses similar methods to that of vertical incision but it leaves no scars, and that it is time saving and less costly.



Overlapping technique results in minimum incision for lifting

The disadvantage is the you do not cut the glandular for repositioning but you only apply overlapping suture and lift up the breasts therefore this lifting effect may not last a long time. Also, we do not reduce the fat tissue so your breasts might look very flat. However, this method is a very popular no matter what the disadvantages are.

Different methods are used for breast ptosis according to the patient’s age, skin elasticity, sagging degree, breast weight, etc. Not one method can be effective for everyone.

Therefore, it is best that you talk to your surgeon to find the best method to your problem.

Unsatisfying factors after correcting breast ptosis

Today, I would like to share about some unsatisfying factors after the mastopexy surgery (breast ptosis, sagging breast lifting surgery)

This is not as common as breast augmentation so people have some knowledge on it from the internet but they are not aware about the dissatisfying factors that may be caused compared to the much knowledge that they have regarding the problem and side effect of breast augmentation.

The first dissatisfying factor is the shape (Peak point is rather flat)

Designing on how to correct the sagging breasts needs much skill and experience as there may be unexpected problems. There are many comments that the breasts do not look good after the lifting.




Usually we ties up the surrounding of the nipple. The nipple needs to point outwards but it stays flat at the peak point of the breast. In such case, we need to minimize the ‘scar’ caused therefore we conduct incision on the diameter of the areola and conduct lifting on the donut-like shape.

In order to prevent this flatness at the peak point, vertical incision is recommended.

Vertical incision helps fix the sagging of the skin tissue, and make the breast look in ideal shape.

If the breast gland does not sag, but only the nipple does, then we do not need to apply the vertical incision...........


The second dissatisfying factor is that you may not like to position of the nipple / areola

The nipple and the areola must be located at the peak point of the breast in order to look good cosmetically. If you disregard the position of the areola and the nipple when they are located towards the bottom of the breast and insert an implant, the implant would make the breast look larger but the nipple would fall below the breast gland facing downwards. This would end up in very unsatisfying shape of the breast.



Also, if the surgeon designs the correction so that the areola is placed too much at the top part of the breast, this would also look weird. (We call this false ptosis)

If you want to position the nipple and areola right at the peak of the breast then the surgeon must estimate how much breast tissue to lift up when designing the procedure. You need to conduct the procedure as planned to make the position correct but this may not always happen therefore leading to further correction. There are more variables if you use an implant.


The third dissatisfying factor is the size of the breast

If you have large original breast, the usage of the implant may make you dissatisfied because your breast has become too big. But if you have small original breasts and use implants, you might be happy with the size but you can be unhappy with the skin elasticity and tightness.

If you have large original breasts, then it would better off to not use the implants when conducting lifting procedure. The implant would lead to added volume that may cause more variables…


Many people think that the “scar” might be the biggest problem but amazingly, there aren’t many complaints regarding scars after the correction. (The scars tend to go away easily after the correction)

There are different types of sagging breasts. The correction method also changes according to the degree of sagging. Today, I would like to share different type of methods in solving the sagging breasts.

Most doctors differentiate sagging by observing the relationship between the location of the areola and the inframammary fold. (We call this Regnault’s classification)

I have shared details on the classification in my blog as well, but many people who have read it cannot actually know how their breasts are. Also, there are some exceptional cases. So, today, I would like to share just two concepts for all of you to understand easily.

Sagginess is caused by two reasons.


The first is when the composition insider the breast sag. The compositions would be tissue cells and fat. The ligament stretch out while holding the composition leading to sagging of the breast.

Let’s call this ‘composition sagging’.

Then there is a case where the outer skin stretches out leading to sagging. Let’s call this ‘skin sagging’. This is caused because the skin is not elastic. In such case, both the nipple and the areola drop downwards as well.



The composition sagging is caused because the original breast is large. The volume of the original breast is large (with much fat) which leads to quicker sagginess. There are times when the composition is sagging but the skin has much elasticity which helps to maintain the form of the breast properly. This usually happens to young women after pregnancy or after diet. The composition drops down but the young and strong skin supports this.

When the composition sags but the skin does not, we call this mammary gland ptosis. If this type of ptosis is not severe, we can use implant to correct it.

We need to move the inframammary fold line downwards therefore need to apply dual plane procedure.

Outer skin sagging without the composition sagging is very rare.

- Those women with large original breasts before pregnancy -

These women usually have composition sagging.
These women after giving birth will suffer from worse composition sagging that also leads to outer skin sagging.



- Those who had small breasts before pregnancy -

These patients do not suffer much from composition sagging after giving birth as well, because of the small original breasts.... But they suffer breasts getting bigger during pregnancy and then the loss of volume after giving birth which leads to loss of skin elasticity.

This is where there is more of outer skin sagging compared to composition sagging.

We need to fill up the inside in order to strengthen skin elasticity. The loss of volume after giving birth is familiar to a balloon with a loss of air… this can be fixed with implant. Breast augmentation is good enough as a solution.

What if your areola is positioned so low that you want to change its location. We can move the areola at the desired spot in this case.





 - A lady with somewhat large breast before pregnancy, but the breast sag after breastfeeding -

This is the most common case. It has in more than half the time. This is when there is composition sagging but not skin because the patient is still young.

As you get older and have more children, the skin starts to sag and the areola moves downwards as well. These patients need lifting correction in order to correct the composition sagging.

If the composition sagging is not severe without areola sagging, then dual plane procedure would be good enough.


As such, we need to know the degree of sagging and its form in order to provide solution for it.


There are many exercises that you can refer to on the internet in order to prevent your breasts from sagging, but in reality these exercises do not help you much.
Exercises are to make your muscles stronger, but sagging is caused when the tie between the muscle and the breast tissue becomes loose. It is not the muscle that is becoming loose.
Therefore, stronger and bigger muscle does not help avoid sagging.

Sagging breast correction

Many do not understand the cause of sagging breasts. If you think that correcting the sagging breasts is the same as lifting the sagging skin on your face, then you are mistaken. Sagging breasts correction is picking up something at the bottom and hanging it at the top. Sagging breast correction, especially for those Korean women who are small and thin, should be differentiated from the classical sagging breasts correction.
First, the implant is put in below the pectoralis muscle by applying the dual plan method and this should benefit the effect of having to be pulled up together to the part above the pectoralis muscle. Second, as the implant goes into the breast, if forms a tightening effect by pulling on the skin resulting in optimized elastic feeling on the breast. Dual plane method for the implant makes you feel that the breast has been pulled upwards. This is very important to those Korean women who are really thin and do not have much fat in the upper body. It is important for the implant to fill up the volume on the upper part of the breast.

Third, we can remove the areola and the nipple to the upper location. This transfer gives enough lifting effect to the Koreans. If you have thick skin and your original breasts are comparably big, then we need to pull up the deep skin upwards to see positive results.
