Today, I would like to share about U-shaped thread for face thread lifting.

There are basically two types of threads, melting one and non-melting one. There is a non- melting thread called aptos, however this is not used today. The threads used today are all melting ones.

The melting threads can also be classifed into two groups, one with hooks and the other without any hooks. The hooks are used for strong lifting. We call the threads without the hooks, mono threads. You might wonder how no hook threads can lift up the skin? However, these threads are also very effective in providing anti-aging effect. The no-hook thread acts as a stimulant and gathers fiberous tissue making the skin come together. The hook is designed for lifting but the thread without the hook is used to tightening.




Most procedures mix the use of threads with and without the hook to provide anti-aging effect. There are many types of threads with hook also. There are hooks that are ties up or shaped like a cone. The most common one is the hook. The hook can be made by cutting or by sticking the cog. So there are many who are concerned with the thread being weak because it is somewhat damaged by the cutting.

There are U-shape and I-shape for cog thread. U-shaped cog has the most strength to maintain the lifting power. The principle behind is one fixation spot is not enough for the tensile strength therefore the fixation is done with the whole thread itself.




There is a procedure where the surface is used for the fixation. This would lead to strong resisting power but if the fixation space is wide, there must be more anesthesia used and there would be more pain with the suage of more substances.
Therefore, u-shaped cog thread is used most commonly.

Managing breast augmentation and its side effects

There are those who are ask questions such as “How do you take care of capsular contracture in this clinic?” when I talk to the patients.

I have felt that many patients are unclear about what the hospital can do if the patient suffers side effects or other complications after breast augmentation as they gather information on the hospital.

Today, I would like to share about a complication that patients are most concerned about, capsular contracture.

1. Can it be avoided? Why does it happen?
Let’s first look into this and then carry on.
Capsular contracture is something that the doctors are still not biochemically clear about to this day.

It happens to some, and it does not to others. It gets formed to some within 6 months and disappears. One might not have any problem for 5 years but then suddenly it gets formed. What is the reason for this? We still do not have an answer.
The incidence rate, referred to a publishment and FDA report, is found to be at less than 20% but Tebetts and others have stated that it is less than 1%. The fact is that the incidence rate of capsular contracture is dramatically low at present than that in the past.

In the past, there were much bleeding during breast augmentation, and it was done above the pectoralis muscle. There was no method or concept such as dual plane. Also, not many textured implants were used. The doctors may not know the cause of the capsular contracture, but they have built up knowledge on how one could avoid it, therefore, limiting its incidence rate. However, we cannot make the incidence rate 0% with the usage of silicone implants. It can be formed although you take much care of your body.

One common cause that most doctors agree on is that the capsular contracture gets formed with infection in the part of the body that we cannot see and the inflammation that is caused. When substance such as an implant goes into the body, it gets polluted with air outside. Such pollutant reacts with the body and the matabolism system causes a capsule that is strong and hard. This capsule which makes the breast hard is called capsular contracture.

The most effective method in preventing the capsular contracture would then be to prevent pollution as much as possible. There are techniques for this, and these techniques are shared in my other posts.

2. Who is responsible for capsular contracture?
Unwanted happening after a surger is called “side effects” or “complications”.
One would argues if complications such as capsular contracture after breast augmentation is medically responsible or if it is something that happens naturally. It is difficult to say that this is something that is caused through medical reasonsing based on the sources and experiments conducted so far. This means that it is not caused due to the surgeons’s act, misdirection, or any other related actions.

Then, can the patient not have the hospital responsible for capsular contracture?
This is not true either because a doctor has a responsibility to do the best he/she can to achieve best result for the patient.



Although, there is no medical fault, the doctor needs to be somewhat responsible for the result. Therefore, he/she has the responsibility to make things right. Therefore, is the capsular contracture is formed, there are invasive methods such as correction, or non-invasive methods such as massaging that can be conducted. We, at our clinic, provide non-invasive methods for free of charge. If correction is necessary, it is free also. However, we charge for anesthesia, medication, and other material costs.

If there is malpractice or medical fault such as leaving the gauze near the implant, not knowing about it, and causing inflammation, I think that the hospital cannot charge the patient for breast augmentation. If the hospital does not react to such problem quickly enough leading to worsening of the inflammation and implant collapse, then this is a medical accident and the hospital must be reliable for compensation as well.
However, this is not the case for capsular contracture. That is the solution to it is vague and the hospital cannot give a clear answer.

3. Does correction help recover from capsular contracture?



When corrrecting a severe case of capsular contracture, we open up the epithelium. A complete removal of the epitheium might be necessary at times. The implant can be replaced as well.

There are patients who ask if the recurrence of capsular contracture can be completely avoided. The recurrence is avoided 90% of the times after the epithelium is opened up.
There are many who search for “capsular contracture” on the internet and think that it is the scariest complication. From my much experience, I believer that it is not the worst complication that one can suffer. Also, your interview with the doctor is very important before the surgery.

Therefore, you do not need to worry or be scared off capsular contracture. This applies to those who are waiting for the breast augmentation and those who already have got one. The assumptions from the internets are much different from statistically known facts.

There was a wave of facial contour surgery in the early or mid 2000’s for about 10 years.
At the time, the nose and eye surgery were popular but there were some doctos who said that they can make your faces smaller and make it look like a westerner’s by shaving off the facial bone, breaking it and gixing it again. Young women became aware of this and it became popular. Many clinics were formed near the Kangnam area.



Some cosmetics clinics conducted surgeries for bone structure problem such as bimaxillary (Two jaw) surgery. Some dentist and cosmetic clinics conducted some inappropriate procedures as well.

There was no end to the crazy demand and such illegal procedures. However, a lot of celebrities and others became more aware of the side effect and problems that were caused. The demand for bimaxillary (Two jaw) surgery started to decrease.





Bimaxillary  (Two jaw) surgery is one that makes your face smaller or changes the structure by breaking  or polishing facial bones. But, Korea is the only country where this operation is demanded much for cosmetic reasons.

The main purpose for cosmetic surgery is to make you look younger, and make your body look better (i.e. antiaging, BOTOX, filler, fat transplanation, breast augmentation, etc) There are cases where people want to change the original facial structure. There was a time when such demand was great in Korea.

The bimaxillary  (Two jaw) surgery should be conducted only in situations that it is necessary and there is no alternative. However, it was conducted without any appropriate reason. Such application of the bimaxillary  (Two jaw) surgery led to much claims from the patients.

One of which is the sagging of the face.

Why does bimaxillary  (Two jaw) surgery make your facial skin sag?



The answer is simple when you know about the bone structure. Bone is one of the many that maintains the shape of the body. It also supports the muscle that helps you move your body.

The bone is to support the facial skin and fat as well. If you conduct cheekbone surgery, the bone at the point where the skin, fat, fascia, muscle meet needs to be broken. In order to break the bone, the skin in the mid face must be removed with the bone broken. Therefore, the skin cannot get back to its orginal location properly. This causes facial skin to sag and your cheek to sag.




What happens if you correct square looking jaw?

The jawbone is suppose to stick outwards so that the masticatory muscle can be placed there as well. But if you cut off the jawbone, many problems start to arise to the skin that holds the jawbone. If there is much bleeding caused here, it could lead to an urgent situation for the patient.

Jawbone correction is conducted to correct the jaw that sticks outwards on both sides. The skin under the bone that is shaved off needs to stay there but the skin rather sags down.


Such problem can be ignored when correcting a square face by just cutting off the jawbone, but the longer the bone that needs to be shaved, the more sagging the skin / muscle becomes.

Facial contour from the front view is important but the edge of the jawbone should not be sharp. The  boundary between the face and the neck needs to be sharp in order for you to look sophisticated but if you shave off the jawbone, this boundary becomes dull due to the sagging skin. You might hear that the correction was successful but it seems that there is still something wrong.

Your skin is more elastic when you are young, so you may not have much proble, But as you get older, your skin loses tension and this leads to sagging of the skin.

Those young patients who go bimaxillary operation during the peak period of 2000~2012 are now in their 30’s~40’s. In a few years time, when they become 40’s ~ 50’s,, they would suffer more problem with porblems stated above.



You cannot restore the bone that has been cut off. Therefore, we need to pully up the skin that slides down with the cutting of the bone. However, the patients are too young to apply face lifting, i.e. bimaxillary operation. Therefore, minimal lifting is required to fix the side effects. Thread lifting would be the best solution.

The thread should be fixed on the temporal region for fixing the sagging effect after the cheekbone surgery. The thread should connect nasolabial fold, and ths cheek skin. This is similar to that of other antiaging methods. The pulling strength depends on how much your skin is sagging.

If the thread lifting is not effective enough, then other methods such as ulcera / ultraformer or accusculpt / fat absoption can be conducted.


Such sagging effect from the bimaxillary  (Two jaw) surgery is caused due to the rold that the bone shaved off plays therefore instead of recovering the bone, we need to lift that sagging skin up. But a you age, the skin loses elasticity and tends to sag again.
Therefore, just like the other anti-aging methods, you need to get repeated treatments.

Getting to know about breast augmentation

Whoever gets on the surgical bed would be scared to get a surgery. This is something normal. The biggest reason for such fear is the pain that you would feel after the surgery.

Today, I would like to share about few things that you can do in order to prevent the pain and achieve a comfortable surgery for the breast augmentation.



There are those sho feel much pain and those who do not. There ar enot who do the feel the pain at all. You need to know the difference between these patients in order to know why there is a difference in pain level.





First, it is the pressuring strength of the implant. The pain is caused through the pressure that the implant sets on the skin and the bone that it is located on. The implant is made to put more volume to the skin but it causes pressure that leads to pain. It is know that the most pain is caused in the periost. The pain recpetors that cover the periost make you feel the most pain.

Second is the trauma or the external injury.In order to get a clear vision, the doctor uses retractor, endoscope and other equipment that physically tough the patient’s skin leading to much pain.

Third reasons is due to the breast skin that wants to resist the strength that make fascia-muscle-tissue strech. If the implant goes into the breast, the input area gets larger. The skin is similar to spandex that resist the power of the person wearing it. Therefore there are many sensing receptors that make you feel the pain.

The fourth reason, is the difference int he level of how each person feels the pain. A hit on the forehead might make some people feel a lot more hurtful than others. To them, it is not right to explain that they would feel less pain with the surgery. Each patient would feel different level of pain.



The first type of pain would be severe with larger implant, and less with smaller implant.
The second type of pain would be severe if the doctor is harsh and not careful. It would be less painful if the doctor is more careful.

The third type of pain would be more severe to those patient that are young because they have tighter skin. The skin sags as you age, therefore the pain would be less for older patients.

If you know what type of person you patient is, you would be able to know what to do so that she feels less pain when conducting breast augmentation.

You should avoid using an implant that is too large. The doctor should be careful when conducting the surgery. The patient’s skin needs to be less tight and she needs to feel less pain.

The third and the fourth reason for the pain are those that we cannot control. But the first and the second reasons are those that we can control.



The doctor should use an appropriate size implant and should be careful not to touch too much on those spots that have pain receptors.

If these directions are met, then the patient does not need to worry about the pain as much.

Basic knowledge of anti-aging

Today, I would like to post on something that many of those who get thread lifting ask about.

There are those who say that thread lifting is very effective at first but the effect goes away very quickly.

Many people know that thread lifting is not a permanent thing and the effect disappears as the face skin sag, but they are also curious about what to do in order to have the effect last for as long as possible.

First, the sagging pattern of the face needs to be suitable for thread lifting. (This is very important)

Second, the thread must be applied at the appropriate spot and it needs to pull the skin in the right way.

Third, the thread needs to be strong and it needs to be durable by not having much straing on it.



There are many things to talk about, but today, I would like to focus on the fixation point and the pulling method.

We put in the thread at the spot above the earflap and send it to the point of the skin where we want to pull up.

We can remove the thread or leave it as it is.

The point where the thread comes out differs according to the target area of which we want to have the surgical effect, but the input spot is always the same, at the upper part of the earflap. What is the reason behind?



The skin where the thread is inputted must be fixed (there needs to be a spot where supports the process). The fixation point needs hard with strong bearing power in order for the the pulling effect to last for a long time.

The temple would be such a spot in the upper face. This area would include the temple as well as the fascia. The fascia is located very close to the skin and is very  tough and does not stretch easily therefore is used as a fixation spot for thread lifting.



Whatever type the thread it, if it does not have a solid fixation spot, the thread would not be able to have enough power to pull the skin. The pulling strength would disperse upwards and downwards.

When the thread penetrates through the skin, it must uniformly penetrate through the subcutaneous fat and SMAS. The penetrating direction needs to be appropriate for the soft tissue masses that form the face.



The end of the thread needs to go straight into the wrinkle line of the target part and the amount of threads used is depended on the depth of the wrinkle.

There are various methods for thread lifting and one cannot say which is better than the other. The doctor’s decion on the method through his / her experience is the best method that can be applied.
