How do you correct asymmetrical eyelids?

If you ask any 100 plastic surgeons and ask them a question, there is one question that all of them have difficulty answering. That question is the one above. The correction of the mismatched eyes, whether it is the first surgery or the re-surgery, is very challenging and stressful on the surgeons.

There are many people who are not aware that the shape and the functions of both eyes of a person are different.

For example, when you walk, both legs act differently. Left leg is mainly used to support your weight and the right leg it used to go forward. The tracks used for running is oval shaped. The runners go in anti-clock wise direction and turn left-wards to support the weight of the runners.

When you look at your hands, the thicknesses of both hands are different. The hand that you use more has more muscles. A lefty would have thicker left hand than the right.

This applies to same to our eyes as well.

There are those who are right-sighted and those who are left-sighted. If you tell a person just to open one eye when she looks into a microscope or a telescope, there are some who close their left eyes and those who close right eyes. Not every person uses both eyes equally. The fact is that the difference of the two eyes starts from here. The two eyes cannot be perfectly equal.

If you corrected the smaller sized eye through blepharoptosis and suddenly made it big, the opposite eye would look lagged and smaller than before the correction. We call this a Hering's law. This happens to 10% of all blepharoptosis correction.

As you can see from the Hering's law, there are many things that can happen after the eyelid correction. Other problems can be caused through anesthetic or other chemicals. Therefore, eyelid correction is thought to be difficult and even more difficult by many surgeons.

1) Why do you have asymmetrical eyelids?

This may be caused by the difference in lagging eyelids. Or it may be caused due to difference in strength of opening your eyes. If you have a tissue damage in earlier surgery, such damage may cause the asymmetrical eyelids as well.

Now a days, there are many patients who received double eyelid surgery (known as Asian double eyelid). These patients go about various clinics due to the asymmetrical eyelids.

If you have a problem with the eyelids, then we need to remove some parts of the skin to balance out the two eyes.

If you have difference in strength of opening the eyes, we need to pull up the weaker side to balance out the strength.

We need to have same environment setting for both your eyes when you open them. For example, if you have a scar on one eye and not the other, we need to remove the scar and make sure that the tissue under the scar is fixed so that it moves freely.

If one eye lags more than the other, we need to find the cause and recover the damaged tissue.

If the double eyelids are different, then we need to see if there are any structural problems and balance out both sides.

2) How do you prevent asymmetrical eyelids

Do not be too greedy about the results from the 1st surgery.
Do the surgery that is recommended by the surgeon.
Do not stick to what you found on-line or the surgery that you are attracted to. Do not force the surgeon to do whatever you know or want.

A surgery is conducted 100% with the hands therefore, each surgeon has his/her own method. A surgeon would like to do a surgery that he/she is confident in. This is another reason why a patient should talk to the doctor that conducts the surgery.

If you think of a surgery just like any other shopping, then the surgery becomes something that is very harmful.

This will be the start of a very bad surgery.

Does the doctor know the details about my eyes?

Is the doctor interested in my eyes and show signs of

how he/she can improve my eyes?

Does the doctor share what is possible

and what is not possible according to what I want?

These factors are more important than the new techniques or new surgery methods. Something else is wrong if the doctor says Yes to everything that you require. A surgery is not committed by a god but by a human being. Also, a doctor must think of various factors that the patient does not think of or consider. In most cases, it is difficult the match the both eyes in the re-surgery.

Good looking nose – Structuring framework – Korean people’s nose

For the Asians, it is more important that we do not damage the framework and necessary that we make it stand out in more healthy and firm way.

Let’s look at the framework of the nose, first.

The upper part of the nose is composed of bone, and bottom part is connected with cartilage. The bone does not move. It does not slip / fall downwards. So if we put something on top of the bone, the nose will stand up higher and higher.

So the basic understanding is that we put silicon implant to make to nose look higher.

If the upper nasal framework, i.e. bone, is bent, we need to correct it.

At times the junction between the bone and the upper cartilage sticks out. If this is severe, we call it aquiline nose (also called a Roman nose or hook nose). We need to remove it or correct it. When this is complete, we do not need to do much more on the upper framework.

Now, we have the lower framework, cartilage left to work on. This part moves about according to how you touch it, so we cannot just put something on top of it. The cartilage may be pushed downwards or spread out on both sides. We need to put something in order for the tip of the nose to stand out, we call this nasal strut graft fixture.

The picture above shows the space between the lower cartilages. This part supports the lower part of the nose and is called the nasal strut. When we do a nose surgery for the Asian patients, we focus on strengthening the framework by supporting the lower cartilage. This is the very first mission in most surgeries.

We need to consider nasal septum cartilage, in such cases. The septum is normally flat so it acts as a supporting measure. If the nasal septum cartilage is strong, one layer is enough to support, however, if not then we need double layer for support.

Nasal strut graft

We complete half of the overall procedure once this nasal strut graft, the strengthening of the lower framework, is complete. The works on the other nose parts are done after the work above are complete.

Communication between the surgeon and the patient

Whenever I talk to my patients, one thing always come up on my head, that communication is very important.

Actually, I think about this every time.

How can the patient understand me clearly?

How can I understand what the patient wants?

This looks simple, but it is very difficult.

A doctor or a surgeon has professional knowledge and thinks that he/she should be more knowledgeable in both learning and experience compared to the patients.

When a doctor interviews with the patients, he/she tends to think in his/her own point of view rather than focusing on what the patient wants, and approaches strategically on sales to provide what the patient wants.

A patient may have some structural knowledge but there are increasing number of cases where the patients gains knowledge from the mass media or the internet, already has the answers, then approaches the doctor.

In such a situation, a patient would hear what she likes about the details communicated with the doctor, and then decide on whether to have the surgery or not.

We call this, doctor shopping.

Therefore, when the doctors meet the patients in the clinics, the doctors do not communicate the facts but tend to look at the psychological aspects of the patients and sell what they want. This becomes more of a sales approach.

However, this is wrong.

If communication is difficult, it is caused by either the doctor does not listen to what the patients wants or that the patient wants to find a doctor who just says what the patients already has in her mind.

Even though, you have a long discussion, this would not lead to anywhere.

There are 3 topics to keep in mind when a patient talks to the doctor about a breast surgery. These are basic but important topics.

1. How large should the size be?

2. Where should the incision be?

3. Where should the implant be placed?

A decision making for breast surgery should be a team decision, i.e. one should come up to the final decision through communicating and thinking together.

A surgeon should measure the upper body and look at the status of the breast to find what implant size to consider in the surgery.

A patient should check the implant and the size by checking the size with the upper body and make sure that the implant size is what she wants.

The implant size will be rechecked and modified, if necessary, during the surgery procedure, and be fit in accordingly.

When deciding on the incision, the patient first shares about her thoughts and shares the reasons as well.

The surgeon looks at the body shape and the problem related with the breasts, considers important factors in the surgery, the type and size of implant, and finally recommends a logical part for the incision.

However, the final decision is based on the agreement between the patient and the surgeon.

The most important part is the surface where the implant goes in. In our country, most patients are thin or have weak upper body so we put the implant mostly under the muscle or in the form of a double plane.

All these processes are decided through communication.

One side cannot make all the decisions by enforcing his/her own thoughts

We look at all situation and possibilities and then come up with the best solution.

This is the core part on the breast surgery interview.

Today, we have a “communication” issue in politics. A conversation is that much important.

This is hopefully a one-time surgery, therefore, breast surgery requires an open communication between the surgeon and the patient, but it is not as easy as we think.

First, we need to be sincere and honest. This requires a lot of effort as well.

Eyelid lift Surgery - Making your eyes look younger

The first sign of aging can be seen through the droopy eyelids.

This is something that you cannot do much about or prevent. As time flies, your eyelids start to cover your eyes. The eyelids have the softest or the thinnest skin in your body. It is natural that your eyelids droop quicker than any other skin do.

So it is obvious that the droopy eyelids is the sign that you are aging.

I am getting old! I have become a grandmother!... can be seen through your eyes.

There are many methods on how we can correct the droopy eyelids. The most representative one is the Double Eyelid surgery (or Asian blepharoplasty).

If you already have double eyelids, we can cut off the drooping part and correct the double eyelids. We call this Eyelid Correction as well.

We would approach this with an out-fold design. Radial wrinkles are formed inside the eyelids, and it becomes more difficult to correct this if the drooping angle is more towards the inner side of the eyelids.

Another problem is when the eyes become hollow. As you age, your eyes become hollow and look like you look older than your actual age.

Before and after Eyelid lift surgery (Blepharoplasty)

Your vision becomes clear when you correct the droopy eyelids (eyelid lift surgery or blepharoplasty). Also, you would frown less leading to less wrinkles and maybe even preventing the wrinkles.

At times we also conduct fat transfer for the hollow eyes.

Also, as you age, it takes more strength to open your eyes. This is due to the natural decline of muscles in your eyes. If you are interested in maintaining your youth and health, we advise you to check the status of your eyelids.

The weaker eyelids, as you age, make you feel:

1) Your eyes are becoming smaller.

2) Your forehead help you open your eyes, therefore causing more wrinkles on your forehead.

3) Your visuals are not clear therefore you frown more.

4) The double eyelids cover your sight making your visuals less clear.

5) Your eyes do not look sharp, and so you look tired.

These symptoms can be evaluated easily.

If the strength in opening your eyes become weaker, droopy eyelids correction may not be enough, you might need Ptosis correction.

As you age, the skin of your eyelids become less elastic and droops down simultaneously your eyelids becoming weaker. You can remove the drooping skin and correct it through the Ptosis correction.
