There was a person who asked about the seroma that could arise after the breast surgery using an implant. I would like to share about this today.

If you put blood sample in the test tube and leave it for a while, the liquid rises and the blood cell stay at the bottom. The clean serum gathered together at a particular place is called seroma.

Small blood vessel damaged leaks plasma and cases seroma. Infection can cause seroma as well.



A hermatoma containing red blood cell is wrong and an abscess containing pus is different.



Seroma is formed due to some kind of trauma on the body. It gets sucked into the body as time passes. This may take days or even weeks. Calcification can be cause after the seroma disappears.

This happens a lot during the breast surgery (mastectomy), abdominal surgery, and reconstruction.

More the difficult the surgery gets, there is a higher possibility of seroma and the more larger the seroma is, it takes longer time for it to disappear.

Also, there is a possibility of second infection as well. Such a large seroma can take years to go away.



In 2012, there was a plastic and reconstructive surgery journal article written by Spear and others. They analyzed 28 cases of late seroma after breast implant. The seroma was formed at an average of 4.7 years after the surgery and most of the cases used biocell textured implant, Allergan’s textured implant.
One can remove the epithelium or replace the implant in order to cure this. There are cases where only the seroma was removed as well. Some used antibiotics for the cure.



96% were found to be cured properly, however we do not know the cause. There were no cases of infection or malignant altercation in the journal that Spear and other wrote.
The first scholar to mention about seroma in breast augmentation was Elizabeth Findlay. Dr. Findlay reported that all cases of seroma were found with the usage of Allergan’s textured implants. The reason for this is that all Allergan’s textured implants have a surface of Biocell that have strong friction and this causes trauma damaging the blood vessel - causing seroma. The seroma found by Dr. Findlay was found 6 months after the surgery (This is why the term ‘delayed’ was used).

From my experience, I think that it is more common to find seroma right after the surgery lasting for 6 months, which is different from what Dr. Findlay found out.
We, at our country, use Polytech, Mentor, Allergan, Silimed, Cervin, etc implants and I don’t think that the seroma formation only applies to Allergan textured implants. There were such cases found in Polytech and Mentor implants as well. Smooth round type implants had the same problem also. There weren’t any cases found in Silimed implants as of yet.


So, I think that instead of calling this “late seroma”, it should be called “Nonabsorbing seroma”.

If the pocket for the implant is larger than the implant itself, the contract-union of the implant and the pocket is not be conducted properly causing much space in between. There would be liquid formed in this space that do not get sucked in for a long period of time leading to the seroma formation.

A certain type of implant from a certain brand do not fit in with the size of an average Korean woman’s rib size therefore there can be a mismatch and empty space between the implant and the body. I think that this could also be a cause of seroma formation. Of course, there needs to be more cases gathered and experiments conducted to know about this with certainty.

Sagging breast correction

Many do not understand the cause of sagging breasts. If you think that correcting the sagging breasts is the same as lifting the sagging skin on your face, then you are mistaken. Sagging breasts correction is picking up something at the bottom and hanging it at the top. Sagging breast correction, especially for those Korean women who are small and thin, should be differentiated from the classical sagging breasts correction.
First, the implant is put in below the pectoralis muscle by applying the dual plan method and this should benefit the effect of having to be pulled up together to the part above the pectoralis muscle. Dual plane method for the implant makes you feel that the breast has been pulled upwards. This is very important to those Korean women who are really thin and do not have much fat in the upper body. It is important for the implant to fill up the volume on the upper part of the breast.

Third, we can remove the areola and the nipple to the upper location. This transfer gives enough lifting effect to the Koreans. If you have thick skin and your original breasts are comparably big, then we need to pull up the deep skin upwards to see positive results.

Today I would like to answer to those who are curious about HIKO(nose thread lift) correction through this post.

There has been a lot of interest and experiments on how to make the nose higher without the usage of a mes. When we talk about cosmetic surgery of the nose, it not only includes making the nose higher but also correcting the shape of the nose. Below are some methods.

There has been continuous demand for cosmetic surgery that does not include the usage of knife, no bleeding but something that is comfortable, cheap and quick in result. This has led to development of many techniques.

Non-surgical methods are less successful but this does not apply to rhinoplasty. i.e. making the nose itself and the nose tip higher, which has been rather more progressive in getting positive results.



HIKO nose correction is one of the methods.

HIKO(Nose thread lift) correction requires the usage of melting thread that is put into the nose through a needle. The thread inside the nose mobilizes the inflammatory cell and starts to melt by itself. The mobilized inflammatory cell makes the surrounding area hard and the scar tissue, i.e. many types of fiber get mixed changing the structure.
This makes the shape of the nose look different.



This is the main essentials of nose lifting, HIKO correction. The silicone is used and the nose rises up, not as much as the usage amount of the silicone., but the silicone leads the change of the surrounding tissue, and finally changing the nose shape.

The degree of change and the time of lasting affect is less than the usage of silicone or transplanting cartilage. However, today, people do not want their nose to look obvious after the correction although the noses get higher therefore, such method of using thread for nose lifting is becoming popular.

If time passes by and the thread melts away and the nose turns back to the original state, this method can be applied again.

This is even more clear with aged patients. High blood pressure or diabetes cannot be cured therefore one needs to take care of the body all the time. Beauty is perceived as the same and people are trying to take care of the body (antiaging or obesity).

There are some cases that need big surgeries. The patient must be clear about how much change he or she want in the body. The more the patient wants, ‘caring or maintaining’ turns to ‘changing’ of the body.

There are many who are aware that caring does not apply to crooked nose correction. But if they want some level of change, caring is the more closer approach that people accept.

It is common to combing thread lifting with other methods for HIKO.

The most representative case would be the usage of the filler.

Filler is effective when you want some level of volume but it is half liquid therefore when it is put into some spot, it tends to disperse to the surrounding area. In opposite, the thread does not give volume but it tends to stay at the injected spot. Therefore there are many cases where both methods are used to complement one another.



There are times where Botox is applied at the nose tip as well.

There is a muscle that pulls the nose tip down at the nose pillar. If you put Botox here, the muscle loses strength and the nose tip will stand up higher. If the thread is applied at this stage, the effect becomes multiple.

Most of the thread used for HIKO procedure is PDO. The thread hydrolyses within 3 months. The lasting effect is for around a year or a year-and-a-half. Filler is about the same, therefore you would need a non-surgical nose reshaping once every year or two. If this fact is acceptable, then high nose correction can be an ideal procedure for you.

When would you need HIKO(Nose thread lift) procedure and when would you need a nose surgery?

If the nose is not too short or crooked, or if it is not too low that the nose can be corrected for cosmetic reasons, then HIKO(Nose thread lift) is recommended.

Today, Botox has become very popular and something that everyone knows about. But, in the 70’s it could not be used as a medical substance. Today, I would like to share about this innovative medicine, botulium toxin.



Botox is a toxin from clostridium botulinum. There are two types of toxin, type A and type B. Botox refer to the type A one. In the early 70’s a ophthalmologist, Dr. Allen Scott used the botulinum toxin to correct cross-eye. This is the beginning of its usage. Dr.

Allen Scott establish a company called Occulinum that got a DFA approval for this medication. Allergan later acquired the company and started to sell botulium toxin A labeled as Botox.



Dr. Carruther, an eye specialist in Canada, with her wife in 1987 found out that botulium toxin applied on the eye spasm led to removal of the wrinkles around. Then they applied it to the forehead of an assistance in the hospital. This was the first case of using botulium toxin on the forehead to remove the wrinkles.

In 1992, Dr Carruther recorded that botulium toxin type A was effective for the wrinkles around the eyes, nasolabial line and wrinkles on the chin. As the same time, there were many organizations in US that announced the clinical testing results of botulium toxin.



As time passed, botulium toxin developed at great speed and was applied together with other anti-aging materials. It was used as a complement for laser treatment and cosmetic procedures that improved on the square type jaw.



The products came out in more variety. There are many company that sell not only Botox but also Dysport, Botulax, Xeomin, Meditoxin, etc.



Botox started to replace various surgical procedures. Those who used to conduct square reluctant surgery or muscle reduction surgery started to use Botox to reduce mastication muscle.



1995, it was known that Botox helped treat the sweating on the hand palm, foot, armpit, etc. This was the turning point for replacement of various surgeries.

Botox usage was expanded to cosmetic area such as retracting calf muscle, correcting nedch and shoulder line, etc.



Its usage covered facial spasm, wry neck, migraine, and other nerve related areas. It can be used when you want a cosmetic surgery to correct your eyes, lips or nose.

The limitation of Botox is the durability and effectiveness. I expect there to be more advancement in the durability and effectiveness of Botox in the near future with more clinical testing and experimentation.

Indian band refers to the wrinkle on the face that looks like the painted line on the face of the American Indians. Some call this a cat wrinkle as well.

The reason for indian band is the thick ligament on the face, ie. the combination of various dangling fixing structures. Indian band is located below the tear trough, and between the upper and midface. The facial part where there are much freckles are at the outer side of the indian band therefore some people think that the indian band is sever due to the freckles.



Indian band also forms similarily shaped wrinkles as the marionette line or nasolabial fold but the shape and state are a little different. Indian band is a lightly formed wrinkle and there is not much fat build on top of it.

Indian band looks black but this is due to the freckles on the wrinkle. There is a coloring from the fore cheek to the outer side and this looks like freckles making the face look darker. This would also look like a dark line at the point where there is a groove. The difference between the indian band and others is that, it is related with the skin coloring and that it is a thin lining. At times, the indian band makes shades on the face making the face look darker. For this reason, the patients want to get it treated.

The most common treatment for the indian band is the usage of the filler. Fat transplantation and coloring treatment are simultaneously carried out as well.



1. Filler;
This is the most effective, quick, simple and cheapest method. The filler used on the indian band is hyalography neuron and is applied lightly on the skin. The problem with this is that when we inject in the filler, we would like the substance to stay at the spot where we want it to be, but the filler disperses into other parts making this non-effective.



In order to avoid the dispersing of the filler due to the pressure around, one needs to prepare tunneling, and remove pressure from the lower ligaments.

2. Threding ;
We us a thread that is inputted vertically for the indian band. Few number of very thin threads are put in through the surface. The collagen gets mixed with the thread and combines with the skin causing wounded skin as well. Such change in the skin tissue resist the indian band. But from my experience until today, threading itself is not enough to prevent indian band. Threading can be done along with the filler as a complement.



3. Coloring ;
Laser toning is used various times in order to treat the freckles. Whitening injection treatment is applied together as well. The coloring treatment can be applied with other methods mentioned above in order to treat and improve on the indian band.
