This does not involve cutting of the skin but folding the muscle-tendon involved in the opening of the eyes. This helps the contraction of superioris muscle.

There are many skin cell tissues at the front sider. The fat, orbicular muscle, etc that loosen up easily when tied together.
But the backside of the eyelid has Muller’s Muscle and superioris muscle that helps tying up more effectively.

If there are other reasons apart from superioris muscle-tendon, then one must also dissect the skin on the eyelid therefore the ptosis correction would also involve dissection. Application of structural nose surgery (diced cartilage)

I explained on the overall concept of the structural nose surgery last time. Today, I would like to share on how the structural nose surgery can be applied to different noses.
When conducting a structural nose surgery, one needs to consider physiological status of the nose, the harmless effect on the skin tissue, and minimized damage on the skin and the tissue.
Let’s compare the nose to a building. When you construct a building, the priority is to build it safely therefore the groundwork foundation of the building should be strong and you also should use the right materials so that the building stays up for a long time.

If you want just a fancy building and do not focus on the basics or the fundamentals, then this could lead to an accident.
Let’s say you want to fix the 매부리코. You want to have this done quick then one would need to grind the nose then place the implant on top with cartilage or derma at the nose tip.


One must use the patient’s skin or tissue in structural nose surgery.
If you took off the cartilage or the bone from the hump, then this should be diced and used to make the nose tip or the nasion higher.
If you need more cartilage, then the cartilage and the dermal grind from the ear can be used and applied on the necessary part. This would help avoid any confusion in the blood circulation or the thinning of the nose skin that could make the inner side visible.

Also, you may think that the nose is very much like the original one.
There are many perceptions on the nose surgery including a negative one. The first reason for this is the silicone implants is too widely used. If it is easy to make out that you had a nose surgery then this means that people who look at you might not all have a positive perception on you.

There are times when the implant is necessary (for ex: when you need large amount of your own skin tissue, but you cannot find more), however, if you aren’t in such a situation, you should try not to use the implant.

We use the diced cartilage because it has longer durability than the lump of cartilage and also because we do not want the cartilage to damage the skin or surrounding tissue.

There are many methods that can be applied on the nose surgery and there are many new methods that are tried today as well. However, the biggest principle does not change.

The principle that the surgery should be structurally safe and the safe environment should be maintained. The physiological balance on the nose should be maintained. These are the principles.

Reasons for crooked nose after rhinoplasty

Many feel that the nose is crooked after the nose surgery.
First thing that you need to know before I start on explaining anything is that nobody has a straight nose. A person has different left and right sides of the face.
This becomes much clear if the silicone implant is used for the nose.

As you can see in the image above, the midline of the face does not align properly with the midpoint of the nose. There are many situations where the nose is not symmetric with the face.

The most common situation is when the silicone implant is placed on top making the bone itself not flat but the surface leaning to one side. At first, it does not look so, but as time passes by, the implant changes its shape according to the shape of the nose and starts to lean to one side. During the nose surgery, one needs to find this leaned side properly and smoothen the bone to place the implant. The nose should not be made to tall or high.

The bone does not look crooked in the upper image but the cartilage and other framework acting structures are bent or crooked. There are many times where the midline of the nose shapes as ‘S’. In such case, one must correct the bent nose then placing the implant.
If the nose ridge is not too low, then you do not need to use the implant.

There are times where the fibula, upper cartilage, cartilage alaris all bend. A lot of effort must be put in if you want to nose to be set straight and tall.
The nose bone must be broken and shaped straight aligning with the midline and use implant to correct and fix the shape.

One of the characteristics of an implant is that it does not mix well with the human body or fat therefore the implant stays separated. If the implant is crooked even by a little, it will be very noticeable from the outside.

If you have a severe case of crooked nose and want to make the nose higher, the result may end up as worsening case of the crooked nose. Also, the implant is separated from the skin and can be noticed easily, therefore, if you have a bad case of a crooked nose, it would be better to use other methods such as filler or dermal fat graft instead of using implant.

Is inframammary fold incision possible for anyone? 

There are many patients who ask about the inframammary fold incision.
Today I would like to share about the inframammary fold incision surgery indication.
The answer to the question is that the inframammary fold incision can be applied at all times. There is no case where the inframammary fold incision should be avoided.
But there are exceptions to everything. If you look at different cases, there are some that state that the inframammary fold is inappropriate.
Let’s use an image.

I would like to show the cases where the scar is covered well and the case where the scar is not covered at all through the image above.

No 1 is normal breasts with appropriate elasticity where the inframammary fold can be covered as well

No 2 is sagging breasts usually after pregnancy where the skin gets loose and the inframammary fold gets completely covered. You cannot see the inframammary fold.

No 3 and 4 are not good for inframammary fold incision. If the patient is willing to forego the possibility of the scar being shown for a long time, then the inframammary fold incision can be applied.

This can be expressed as such. If there is no volume in the breast and the skin is tight, then when the implant goes in, there is much pressure pulling it. The scar on the inframammary fold will be made bigger due to this pulling pressure. Axillary fold incision and areola incision are recommended for this type of situation.

The image above is to explain on the sagging breasts.

No 1 is a severe case of sagging breasts but the overall structure of the breast is maintained. Dual plane and breast implant augmentation can lead to good results. Inframammary fold incision is the best options for dual plane surgery on sagging breasts.

No 3 is less severe case of sagging breast with less volume as well. Breast augmentation would be good enough in such case and the incision spot does not matter.

No 2 is a severe case of sagging breast with the structure being lost. Breast correction such as lifting is necessary and the best incision spot would be the areola.

Above is the illustration of the constricted and tuberous breast. Inframammary fold incision is recommended for such cases.

The reason is as the following.
The ligament and other parts of the body that form such shape of the breasts are very tight. The implant put into the body cannot extend the skin surrounding it. Therefore, one must break through all the tissue that is tightly formed on the bottom side of the breast. If this is not released properly enough, the implant cannot be put in properly and the breast would not look good also.

Such process would require inframammary fold incision.
The patients could make decision on the incision spot according to the psychological pressure or according to the preference. However, the more importation considering point should be how the breast looks.

When deciding on the incision spot, one should consider,
- Is the inframammary fold easily visible? (can the scar also be hidden or covered?)
- How extendable is the breast skin? ( the more it extends, less the scar)
It would be good to remember the two statements above (although those aren’t the only decision makers)

Blunt nose tip correction is done to make the disoriented nose tip sharpened and to close the distance between the two nose wings so that the nose does not look too wide. It is to make the nose more cosmetically better looking.

This is why we need to manipulate the cartilage and also make the nose taller or higher in order to get positive effect.



In order to make the nose higher, one needs to use the septum cartilage to make the pillar. Another good method would be to use the derma that was removed to reduce the nose wing width, and place the derma on the tip of the nose to make the nose higher.

Let’s say that there is a person who weighs about 60kg and is about 150cm tall.  If the person with same weight of 60kg is 170cm tall, then although the two weigh the same, the taller person would look much thinner. We try to make the blunt nose taller in same aspect.
