Deciding on the upper body shape and breast implant

When I counsel for the breast surgery, the very first thing I consider is the upper body shape.

The upper body is the background of the breast, therefore, it is important to see what type of upper body shape a person has.

In some aspect, it is like considering with material of paper you would like to draw a painting on.

In the past, there wasn’t many choices in selecting the implants. We only looked at or considered the cc size and selected the volume.

Now, we have many types of implants that we can select from. It is not only the volume size but also the width, length, and depth of the implant that we can choose according to the body shape and size of each patient. Therefore, breast surgery counseling has become more precise and time consuming.

There are many methods on how we can analyze the body shape or form. There is one method that I have come up with. I call this a Triangle scheme.

The distance from the midpoint of the two clavicles to the nipples, and the distance between the nipples are measured. When all the points are connected, this becomes a triangle. We look at the shape of the triangle to analyze the body form or shape, and then decide on the type of the implant.

Many people have the normal triangle shape. The figure is about 50%. This is the most natural and closest to perfect ratio shape. We recommend round base water drop implant in such case. Round base means that the width and the length of the implant are equal.

The isosceles triangle shape is called the Tall Type. We choose tall base water-drop implant for such case. Tall type means that the length of the implant is longer than the width.

It the triangle shape is wide, we choose a wide back water-drop implant.

As in the picture above, the Tall Height implant is used for those who have tall triangle upper body shape. The Short Height implant is used for those with wide triangle upper body shape.

Such drawing figure (i.e. triangle) helps to decide the implant necessary for the patients quickly.

Of course, this is not the only procedure in making the final selection of the implants. A person’s body is more complicated than one thinks it is. Some people have bird chest, sharp chest, drooping breast, widely separated breast, lifted breast, funnel chest, cancer treated breast, etc.

The implant used must be uniquely fit for such situations and also needs to be in align with what the patient wants, therefore, the measurement cannot be the only decision making factor.

The implant selection depends on:

Upper body measurement + Requests from the patients +
Surgical / Technological factors

The 3 factors must be considered simultaneously in order to make decision on what implant to use.

How do you correct wide separation of breasts?

There are know-hows on how we go about correcting wide separation of breasts.

We can categorize with widening of breasts into two categories.

First, is the inner part in which the cleavage is wide, and the second, is where the distance between the two nipples.

If the sternum between the two breasts is wide, we can put a implants that minimizes the look of margin to margin separation through detachment.

In other words, we call it removing the “expressway”.

This solution can be applied with a little care from the doctor.

However, if there is a much distance between the two nipples, more complex solution is necessary.

In such situation where the distance between the nipples is wide, we can put implants in the inner part. The median of the implants will be placed more inwards compared to the location of the nipples and as a result, your nipples to face towards the outside.

This will not be a beautiful looking breast.

In order to lessen the distance by moving the placement of the nipples and areola, one can surgically remove the current placement by cutting and replacing. However, this leads to many scars and much possibility of reshaping in the areola.

In such situation, we consider a lot of factors and try to solve it case by case.

If the distance between the nipples is wide and the location of the nipples is rather low, we can put in implants to fill up the space and to remove the expressway.

If the nipples are located high and wide, we cannot remove the expressway, but we can use small amount of water-drop implants to prevent sticking out effect so that the after procedure look is more natural.

If your breasts are splayed, we can perform splayed breast correction and at the same time replace the nipple, areola position to make the distance closer.

The wide distance is also related to having a pigeon chest; however this is not always the case.

There are some complications. In order to make the matters clear,

Pigeon chest refers to breast bone – ribs connecting costal cartilage, the center part of your chest pushing outwards from your body therefore, making the upper part, the bone structure, look round.

Wide breasts do not refer to the bone structure but it is a case where the breast soft tissue growth happens far apart from one another.

Therefore, pigeon chest cannot be controlled with breast surgery. One cannot perform surgery on the bone.

But, we can perform breast surgery to make one’s widened breasts look better.
